Vacation Care Permission Form
Please open the relevant details for the date of your child's attendance, then ensure you complete the below form for the relevant day/s. 
Children without signed excursion permissions will not be able to attend.

Terms and Conditions

  1. I acknowledge and accept that this form serves as my official written permission for my child to leave Everton Park SS OSHC (EPOSHC) to attend the specified excursion.

  2. I acknowledge and accept that my digital signature is equivalent to my written signature and authorization.

  3. I acknowledge and accept that I am listed on my child's Enrolment Form as a Parent or Authorised Person.

  4. I have read all the provided details and acknowledge and accept that I can access the Excursion Risk Assessment at the Service.

  5. The contact details, including all emergency contact information, listed on my child's Enrolment Form are up-to-date. If not, I have provided the updated contact details above.
